What is ophthalmoscopy and what can it show?

Ophthalmoscopy is an examination of the internal structures of the eye using an ophthalmoscope or a diagnostic lens. During the procedure, the doctor shines a light through the pupil and evaluates the tissues in the back of the eye. This method helps quickly obtain important information and determine whether routine observation is sufficient or whether further testing is needed.

In clinical practice, fundus examination is used in primary diagnosis, preventive care, monitoring patients with chronic diseases, and post-treatment follow-up.

What eye structures does the doctor evaluate?

Symptoms that require an examination

You should schedule an appointment if you experience blurred vision, flashes, floaters, fog, image distortion, eye pain, a feeling of pressure, loss of a portion of your visual field, or a sudden deterioration in your health. Even if the symptoms resolve, it's best to discuss them with your doctor, as some changes may be intermittent.

Preventive fundus examination

This is necessary for people over 40, as well as patients with myopia, diabetes, hypertension, and a hereditary predisposition to retinal and optic nerve diseases. It is also recommended before certain ophthalmological procedures and when choosing a treatment plan.

Regular examinations help detect changes before significant complaints arise. This is especially important if a person already knows about a chronic disease, but their vision remains stable.

Monitoring Chronic Diseases

Direct and Indirect Ophthalmoscopy?

Direct ophthalmoscopy helps quickly assess central structures, including the optic disc and macula. This format is often used during the initial examination to obtain a basic understanding of the condition of the posterior segment of the eye.

Indirect ophthalmoscopy provides a broader field of view and helps better assess the peripheral retina. This option is especially useful when tears, peripheral dystrophies, the consequences of trauma, or changes that extend beyond the central retina are suspected.

Ophthalmoscopy with pupil dilation

In some cases, the doctor performs the examination after dilating the pupil. This approach is called mydriasis and helps obtain a broader field of view. This is especially important when assessing the peripheral retina, clarifying the condition of the macula, or not missing subtle changes.
