What is an inguinal hernia?

An inguinal hernia is a pathology in which fragments of internal organs come out through a weakened anterior abdominal wall in the inguinal canal. At first, the neoplasm may be insignificant and not cause serious discomfort. Therefore, most often, patients pay attention to the problem in cases where it already significantly reduces the quality of life.

Due to the anatomical features of the body, inguinal hernia is diagnosed mainly in men. Without timely treatment, the pathology progresses and leads to complications that are dangerous to health.

What is an inguinal hernia?

Types of inguinal hernia surgery

There are several surgical techniques aimed at effective and safe removal of inguinal hernias.

Operation Liechtenstein

The Lichtenstein operation is an open hernioplasty, during which the surgeon strengthens the walls of the inguinal canal by installing a special synthetic mesh implant. The technique is universal and is used for almost all types of inguinal hernias, regardless of their size and shape. The Lichtenstein operation in standard cases lasts from 30 minutes to an hour and a half.

Laparoscopic inguinal hernia repair

Laparoscopy is a minimally invasive surgery. The essence of the method is that access to the hernia area is achieved through several small punctures in the abdominal wall, each about 5–10 mm long. A miniature video camera and surgical instruments are inserted through the tiny holes. The surgeon carefully removes the hernia and strengthens the weakened tissue using a mesh, similar to the Lichtenstein method, but inside the patient's body.

The advantages of the laparoscopic method are the rapid recovery of patients, the absence of pronounced scars and a lower risk of possible complications.

Traditional open operations according to Bassini and Shouldice

Bassini and Shouldice operations are traditional methods of hernia treatment aimed at strengthening the inguinal canal. They are most often used in complex clinical cases when, for certain reasons, minimally invasive techniques cannot be used.

The Bassini operation involves strengthening the inguinal canal by suturing the patient's own tissues. The Shouldice operation additionally uses a mesh implant. Both methods require open access, so the surgery is performed under general anesthesia or using spinal anesthesia.

General information about the procedure

Causes and Symptoms of Inguinal Hernia

The key causes of inguinal hernia:

  • Genetic predisposition: having first-degree relatives suffering from this pathology increases the risk of developing the disease
  • Excessive physical activity: lifting weights, intense work, regular exercise that overloads the abdominal muscles
  • Obesity: excess weight increases the load on the anterior abdominal wall, contributes to increased pressure inside the abdominal cavity and provokes weakening of the muscular frame
  • Chronic prostate adenoma: as a result of an enlarged prostate, urination becomes difficult, the man is forced to strain to empty the bladder, which also leads to an increase in intra-abdominal pressure

The symptoms of an inguinal hernia depend on the size of the formation, localization and development of complications. The main signs of the pathology:

  • The appearance of a rounded formation in the groin area, which increases with straining, coughing, and physical exertion. At the same time, in a lying position, the protrusion may become smaller or disappear completely
  • A feeling of heaviness, distension, tingling or burning at the site of the protrusion: symptoms worsen when walking, lifting weights, playing sports
  • Disruption of the gastrointestinal tract: when the digestive system organs are compressed by a hernia, constipation, bloating, and a feeling of discomfort may occur

When the first alarming symptoms appear, you should consult a doctor for examination and treatment.

Diagnostics of inguinal hernia

The main methods for diagnosing an inguinal hernia:

  • Examination: the doctor listens to the complaints, examines the patient's groin area for the presence of a pathological protrusion. If a neoplasm is detected by palpation, the surgeon evaluates its consistency and degree of pain
  • Ultrasound: an informative and accessible method for diagnosing inguinal hernias. Allows you to detect even small, still unnoticeable to the naked eye protrusions. Using an ultrasound machine, the doctor evaluates the structure of the tissues, determines the size of the neoplasm and its contents, as well as signs of inflammation of the surrounding structures
  • CT: used to obtain a three-dimensional picture of the abdominal cavity and pelvic organs. Helps to clarify the location of the hernial sac, identify strangulation or intestinal obstruction, differentiate inguinal hernia from other pathologies
  • MRI: allows you to obtain information about the condition of soft tissues, blood vessels and nerves

Indications for surgery

Main indications for surgical intervention:

  • A hernia that regularly increases in size: even small neoplasms can lead to serious complications over time. Conservative methods help to temporarily relieve pain, but do not stop the progression of the disease. Only surgical intervention can completely get rid of a hernia. The sooner the operation is performed, the less health risks
  • Physical discomfort: pain of varying intensity, a feeling of heaviness in the affected groin area, increasing with exertion or straining.
  • High risk of complications for the patient's health and life
  • Recurrence of a previously operated hernia

The doctor makes the decision on the need for surgical intervention individually, taking into account the clinical picture, condition and medical history of the patient.

Contraindications to surgery

Herniectomy is not performed if the patient:

  • Severe general condition: severe weakness, high body temperature, decompensated chronic diseases
  • Acute renal failure: a sharp decrease in kidney function to a critical level, accompanied by metabolic disorders and accumulation of toxic products in the body, threatens serious complications in the postoperative period
  • Malignant neoplasms of any localization: there is a risk that surgical intervention will accelerate the spread of tumor cells through the bloodstream and lymphatic pathways, stimulating metastasis
  • Blood clotting problems

Most contraindications to surgery are temporary, so it is only necessary to postpone the intervention until the health condition is restored.

Preparing for surgery

Before the operation, it is necessary to consult a doctor. The specialist will study the anamnesis in detail, clarify the presence of concomitant diseases, assess the state of health, conduct an examination and determine preoperative diagnostic measures. The doctor will also tell the patient about the nature of the upcoming operation, possible risks and complications, methods of pain relief and postoperative care.

A few days before surgery, it is recommended to exclude from the diet foods that promote gas formation, intestinal irritation and increased load on the liver and kidneys. Doctors advise limiting the consumption of fatty foods, alcohol, strong tea and coffee, spices and salt. Patients are also strongly advised to quit smoking, since nicotine negatively affects blood clotting, slows down tissue regeneration processes, and increases the risk of postoperative complications.

List of tests and examinations

To assess the functional state of the body's organs and systems and identify contraindications to surgical intervention, the patient is prescribed the following tests:

  • General and biochemical blood analysis
  • Coagulogram
  • Determination of blood type and Rh factor (if this information is not available)
  • ECG
  • Chest X-ray
  • Ultrasound of internal organs

Progress of the operation

The operation to remove an inguinal hernia takes place in several stages:

  • Pain relief: general anesthesia - complete loss of consciousness of the patient; spinal anesthesia - the introduction of an anesthetic drug into the spinal cord, which leads to a temporary loss of sensitivity in the lower part of the body, during the entire procedure the patient remains conscious. The choice of method depends on the patient's health characteristics and the complexity of the upcoming operation
  • Gaining access to the hernial sac
  • Reduction of the hernial sac: the doctor carefully places the protruding internal organs back into the peritoneum
  • Strengthening weakened muscles and ligaments of the inguinal canal using mesh implants or the patient's own tissues by suturing the muscles and aponeurosis
  • Closing the surgical wound with special absorbable threads, eliminating the need for subsequent removal of sutures

After the operation, the patient receives a number of recommendations that must be followed for successful recovery.

Rehabilitation after surgery

Rules that the patient must adhere to during the rehabilitation period:

  • Bed rest in the first days after surgery to minimize stress on the operated area of the body and prevent complications
  • Limitation of physical activity: you should avoid intense exercise and sports until the body has fully recovered
  • Regular antiseptic treatment of wounds: systematic performance of manipulations helps prevent the development of infectious processes, as well as accelerate the regeneration of skin and soft tissues
  • Taking analgesics to reduce pain: the type, dosage and duration of taking the drugs is determined individually, depending on the complaints and condition of the patient
  • Avoid prolonged exposure to the sun, visiting saunas and solariums, swimming in pools and open water

To monitor the recovery process, the patient should visit the attending physician regularly. You should urgently seek medical help if severe pain, bleeding or tissue swelling occurs.

Complications after surgery

Postoperative complications that patients may encounter:

  • Bleeding: may occur both during surgery and in the early recovery period. This is a rare complication that occurs in the case of unskilled intervention or in the absence of high-quality preoperative preparation
  • Pain syndrome: caused by tissue injury, irritation of nerve fibers and circulatory disorders. Anesthetics and physiotherapy procedures will help reduce discomfort
  • Hematoma: a collection of blood in tissues formed as a result of damage to blood vessels during surgery. Hematomas usually disappear on their own within 7-10 days
  • Wound infection: manifested by severe pain, increased body temperature, reddening of the skin around the suture, discharge from the wound. Postoperative antibiotic therapy is used to prevent infections
  • Hernia recurrence: the risk of recurrence of a hernial protrusion depends on the complexity of the case, the qualifications of the surgeon, compliance with the doctor's recommendations, and the individual characteristics of the patient's body

I express my deepest, most sincere gratitude to the K+31 clinic and personally to its highly skilled surgeon, Oleg Ivanovich Yudin! I want to share my amazing experience of treatment at your clinic. I had a gallbladder removal, and the results exceeded all my expectations. Oleg Ivanovich, you are a magician! Your professionalism, calm confidence, and attentive attention to all my questions before the surgery completely dispelled any fears. Thank you for your golden hands and sensitive heart. I especially want to highlight your unique approach to anesthesia. I couldn't have imagined such a gentle anesthesia! I woke up easily, without any unpleasant consequences. And the most incredible thing is that just two hours after the surgery, I was fully conscious, feeling great, and already participating in an online meeting! For me, this is the main indicator of the precision work of the entire team. A huge thank you to all the medical staff in the surgical department: the attentive and kind nurses, anesthesiologists, and orderlies. You surrounded me with such care that my hospital stay felt more like a vacation. I can't help but mention the amazing conditions in the ward: modern equipment, cleanliness, comfort, and attention to detail create an atmosphere that in itself promotes recovery. And, of course, thank you to the department managers and administrators. You are always available, and all issues are resolved quickly, efficiently, and with unfailing kindness. K+31 Clinic is an impeccable standard of medicine, where the patient, their comfort, and their health come first. I recommend you to everyone I know and wish you prosperity with all my heart!
