What is mesothelioma?

This is the name given to a malignant tumor that develops from the mesothelium—the thin membrane lining:

  • Pleura
  • Peritoneum
  • Pericardium

This is a rare oncological disease, and malignant mesothelioma grows along the surface of the membranes rather than as a single nodule. In oncology, this tumor is considered difficult to detect.

Characteristics of the disease

The disease develops slowly and may manifest itself decades after exposure to asbestos. However, mesothelioma quickly spreads through the membranes and causes effusion—a collection of fluid in the cavity. The latency period between exposure to asbestos and the first symptoms often reaches thirty to forty years, which is one of the reasons for its late detection.

How does mesothelioma differ from other cancers?

Unlike typical tumors that grow in a nodule, this tumor spreads in a sheet and completely envelops the membrane. This is why malignant mesothelioma is difficult to diagnose early and requires a special approach.

Types of mesothelioma

The disease is classified by the lining of the lungs it affects. Symptoms, treatment, and prognosis depend on the location, so the types of mesothelioma are analyzed separately.

Pleural Mesothelioma

This tumor is the most common and affects the lining of the lungs. Pleural mesothelioma causes shortness of breath, chest pain, and pleural effusion; it is essentially a cancer of the pleura, closely related to asbestos. This type of pleural cancer remains undetected for a long time.

Peritoneal Mesothelioma

In the abdominal cavity, the disease behaves differently. Peritoneal mesothelioma develops in the lining of the abdomen and presents with bloating, pain, and ascites. When it spreads, it leads to carcinomatosis—multiple lesions throughout the peritoneum.

Pericardial Mesothelioma

The rarest form affects the pericardium. Pericardial mesothelioma disrupts heart function and causes shortness of breath, pain, and fluid accumulation around the heart.

Causes and risk factors

The main culprit of the disease is known, but it doesn't act immediately. Let's look at what increases the risk of developing this tumor, as mesothelioma rarely occurs without a cause.

Asbestos Exposure

The key cause is inhaling asbestos dust. The link between asbestos and mesothelioma has been proven: the fibers remain in tissues for decades and trigger the malignant process. The most dangerous are the finest fibers, which penetrate deeply into tissue and are almost never eliminated by the body.

Occupational Risks

Construction workers, shipyard workers, insulation workers, and anyone else exposed to asbestos in their work environment are at the highest risk. The risk persists for decades, and mesothelioma can manifest itself many years later.

Other Possible Factors

Less commonly, the disease is associated with previous chest radiation exposure and certain mineral fibers. Hereditary factors only slightly increase susceptibility, but even without them, mesothelioma is possible after exposure to asbestos.

Manifestations depend on which lining is affected and remain vague for a long time. Early symptoms of mesothelioma are easily mistaken for bronchitis, fatigue, or stomach problems.

Early Signs

Initially, weakness, mild shortness of breath, weight loss, and a low-grade fever are common. These mesothelioma symptoms are nonspecific, so the disease is often detected late.

Symptoms of Pleural Involvement

When the pleura is affected, shortness of breath, chest pain, and a persistent cough become increasingly common. Pleural mesothelioma compresses the lung and causes pleural effusion, and the pleural tumor gradually restricts breathing.

Symptoms of Peritoneal Involvement

Peritoneal involvement causes bloating, heaviness, and pain in the abdomen, along with an increase in its volume due to ascites. In this location, peritoneal mesothelioma is often accompanied by nausea, bowel disturbances, and weight loss.

Pericardial involvement causes heart pain, arrhythmia, and severe shortness of breath. Here, mesothelioma symptoms are especially dangerous: fluid around the heart interferes with its contraction.

It's impossible to diagnose the disease based on complaints alone. Mesothelioma diagnosis is step-by-step, from imaging to microscopy, and an oncologist confirms mesothelioma only morphologically. Most often, this is pleural mesothelioma, less commonly peritoneal mesothelioma, which is found during an abdominal examination.

Radiation Diagnostics

The decisive step is a biopsy of the pleura or other membrane followed by morphological examination. This distinguishes mesothelioma from metastases of other tumors, so diagnosing mesothelioma without diagnosing pleural tumors at the cellular level is considered incomplete.
