What is palliative care and who needs it?

Palliative care is a special approach in medicine aimed not at curing the underlying disease, but at completely eliminating or maximally alleviating distressing symptoms, improving the quality of life of the patient and his or her loved ones. Its goal is to help the patient live as actively and comfortably as possible, despite a serious diagnosis. Palliative care focuses on maintaining the quality of life here and now.

The word "palliative" itself comes from the Latin "pallium" - pallium, and also from the French "palliatif" - palliative, which means a cloak or cover. This reflects the main principle of palliative care: just as a cloak protects from bad weather, palliative care creates protection from the manifestations of the disease, alleviating suffering and improving the quality of life, but does not aim to cure the disease, since this is impossible.

Palliative care is needed by patients with various incurable diseases, when therapy for the underlying pathology is impossible or inappropriate. This applies not only to oncological diseases, but also to severe cardiac, renal, hepatic failure, neurological degenerative diseases, terminal stages of chronic somatic pathologies and other conditions that significantly limit life.

What is palliative care and who needs it?

Basic principles of palliative care

The essence of palliative care is based on key principles:

  • Relieving the patient's suffering. Relieving pain, shortness of breath, nausea, anxiety and other distressing symptoms is the primary goal of therapy.
  • Maintaining activity. Palliative care professionals aim to maintain the patient's functionality and involvement in life as much as possible.
  • Recognition of life and death as natural processes. Well-organized palliative care helps seriously ill patients change their attitude towards death, consider it as one of the inevitable stages and accept it with dignity
  • Individual approach. The basis of assistance is respect for the patient's personality, values ​​and wishes. Everyone may have their own wishes, for example, to stay at home in their own home or go to a hospice
  • Multidisciplinary. A team of specialists (doctors, nurses, psychologists, social workers, physiotherapists) work together to solve all the patient's problems
  • Support for loved ones. It is extremely important to help the family with care, psychological support during a serious illness and during the period of loss

When is palliative care recommended

Palliative care is indicated not only in the terminal stage. It can and should be started at earlier stages of a serious disease, as soon as severe symptoms appear that reduce the quality of life, or when it becomes clear that the disease is incurable and progressive. Early inclusion of palliative care often allows for better control of symptoms, improves psychological state, and can even positively affect the prognosis. In oncology, it can be used in parallel with antitumor therapy (for example, chemotherapy).

Diseases requiring palliative support

Palliative care is recommended for the following pathologies:

  • Oncological diseases (cancer) in the late stages or when the treatment is ineffective
  • Terminal stage of chronic heart failure
  • Severe chronic renal and hepatic failure
  • Terminal stage of chronic obstructive pulmonary disease (COPD)
  • Progressive neurological diseases: amyotrophic lateral sclerosis, multiple sclerosis, Parkinson's disease in the late stages, dementia
  • Terminal stage of HIV/AIDS
  • Genetic diseases with a severe course

Modern methods of palliative care in "K+31"

The specialists of the palliative care department of the K+31 clinic have at their disposal a wide range of methods aimed at alleviating symptoms and improving the patient’s condition.

Molecular genetic testing

This is one of the key diagnostic methods in modern oncology. It allows to identify specific mutations in the tumor for the selection of personalized targeted therapy or immunotherapy, which is especially important for inoperable or metastatic forms of cancer, when standard treatment is ineffective. This gives the doctor the opportunity to achieve control over the disease, reduce the intensity of symptoms and prolong the active life of the patient.

Chemoembolization of tumors

This is a minimally invasive procedure in which high doses of chemotherapy drugs are injected directly into the blood vessels that feed the tumor (for example, in the liver), blocking them. This allows for local action on the tumor, minimizing systemic side effects, reducing the size of the tumor, and relieving pain and other symptoms.

Radiofrequency ablation of metastases (RFA)

Under visual control (CT, ultrasound), an electrode is inserted into the tumor site, generating high-frequency currents that destroy cancer cells by local heating (up to 50-100°C). This method is used to destroy metastases in the liver, lungs, kidneys, and bones. It is relevant for patients for whom surgery is contraindicated.

Implantation of venous infusion port systems

The method involves installing a special port with a catheter into the central vein under the skin. Such an intervention is extremely important for patients who receive intravenous infusions (chemotherapy, painkillers, nutrient solutions) for a long time. It allows avoiding multiple painful vein punctures, reduces the risk of complications (phlebitis, infiltration) and makes the therapy more comfortable. Port replacement is rarely required. The design serves for more than five years.

Installation of stents

When the lumen of hollow organs (esophagus, bile ducts, trachea, bronchi, intestines, ureters) narrows due to pressure from a growing tumor, self-expanding stents are installed - expanders that serve as a frame. This procedure allows you to quickly restore patency, eliminate life-threatening conditions, such as suffocation, return the ability to eat, normalize the outflow of bile or urine.

Forms of palliative care at K+31

We offer various options for providing palliative care for seriously ill patients:

  1. In-person consultations with doctors (oncologist, therapist, neurologist, psychologist) at the palliative care center
  2. Stay in the day hospital of the palliative care department for procedures (chemotherapy, blood transfusion, RFA, port installation) with the possibility of returning home
  3. Placement in a hospice with round-the-clock care, pain control, psychological support and help with everyday issues.
  4. A visit by a visiting palliative care service to provide care at home (monitoring the condition, adjusting the pain relief regimen, dressings, psychological support)
  5. A visit by an emergency team in urgent cases - to relieve an attack of pain, including using potent drugs, or transport the patient to a hospital to provide therapeutic support

Use of transfusion equipment

In a palliative care unit, the following are performed as indicated:

  • Hemotransfusions (transfusion of blood and its individual components) – necessary to correct severe anemia, thrombocytopenia after chemotherapy or blood loss
  • Plasmapheresis/hemosorption – cleansing the blood of toxins in case of liver and kidney failure or severe intoxication
  • Infusion therapy – elimination of dehydration, electrolyte imbalances by administering solutions, as well as parenteral nutrition when normal

Palliative care procedure

  1. Consultation. An initial appointment with a doctor (often a therapist or oncologist) is necessary to assess the condition, determine the intensity of symptoms and the need for palliative care - its scope and focus
  2. Plan development. A team of specialists, together with the patient and his family, develops an individual therapy plan, including drug pain relief, non-drug methods, psychosocial support, and, if indicated, invasive procedures
  3. Plan implementation. Providing care in the chosen form (outpatient, inpatient, at home) with regular monitoring of effectiveness and adjustment of the plan if necessary
  4. Family support at all stages of palliative care. Our specialists inform the patient's relatives about the peculiarities of the disease, teach them how to care, and provide psychological assistance

Combating complications during palliative care

Palliative care cannot help cope with an incurable disease, but with properly organized therapy it is possible to solve another important task - to avoid severe complications. The most common among them are:

  1. Acute pain syndrome. To relieve it, a three-stage pain relief scheme, blockades, neurosurgical methods are used
  2. Respiratory failure. Oxygen therapy, non-invasive ventilation of the lungs, drug therapy for dyspnea are carried out
  3. Nutrition disorders and cachexia. Dietary support, nutritional counseling, enteral or parenteral nutrition of patients are provided
  4. Nausea and vomiting. The patient receives modern antiemetic drugs with a minimum of side effects
  5. Ascites, pleural effusion. Punctures and drainage are performed to remove fluid. Pleurodesis can be performed - artificial tissue fusion to prevent air or fluid accumulation
  6. Bedsores and fistulas. Proper hygienic care, the use of high-quality dressings and their regular replacement help to avoid damage to soft tissues even with limited mobility
  7. Anxiety, depression. An incurable disease entails inevitable psychological difficulties. Psychologists, psychotherapists, psychiatrists can help cope with emotional disorders. Antidepressants and anxiolytics can be prescribed as indicated
