Indications for hospice admission

Hospice care is palliative, it is provided in a specialized institution when the disease is in the terminal stage, and treatment is no longer effective. The main emphasis is on comfort, care, respectful attitude and help in the most difficult days.

Palliative care can be received even by those who continue to receive treatment. In a hospice, a person is surrounded by attention when treatment is completed, and it is important to provide him with peace, safety and decent conditions for the remaining period of life.

Documents required for admission to a hospice

To register a patient at a medical center, you need to prepare:

  • An extract from the medical history - with a detailed description of the diagnosis, stages of treatment and the current condition of the patient
  • A conclusion from an oncologist or attending physician - that active treatment is ineffective or impossible
  • A referral for palliative care - usually issued at the clinic at the place of residence
  • The patient's passport and compulsory medical insurance policy - if free accommodation is provided
  • Consent of the patient or his legal representative to receive palliative care and conditions of stay
  • A power of attorney or documents confirming guardianship (if necessary)
