Leukoplakia

Leukoplakia is a pathological change in the mucous membranes, in which white spots or plaques appear on their surface.

The presence of a white spot is not cancer. It is a precancerous condition. If left untreated, the condition can develop into cancer. If you notice an unusual spot on the mucous membrane, consult a doctor immediately.

What is leukoplakia?

What is leukoplakia? Simply put, it's a white spot that can't be scraped off or washed away. This is the body's way of protecting the mucous membrane from irritation by building a thick stratum corneum.

Leukoplakia of the mucous membrane can occur anywhere: in the mouth, on the cervix, vulva, bladder, or larynx.

What is leukoplakia?

Causes and risk factors

Causes and risk factors

Prolonged exposure to an irritant is usually required to initiate the keratinization process. The causes of leukoplakia are a combination of trauma and immune system issues.

Chronic mucosal trauma

In the oral cavity: sharp edges of decayed teeth, poorly fitting dentures, smoking (thermal and chemical exposure).

In other areas: constant friction, exposure to aggressive environments, or uncomfortable underwear. The tissue simply doesn't have time to recover and begins to "roughen" to withstand the load.

Inflammatory and infectious factors

Inflammation is an ideal environment for this disease. Those at risk include people with chronic stomatitis, gingivitis, and those who have had sexually transmitted infections.

Hormonal, immune, and metabolic disorders

The body's internal state determines the mucous membrane's ability to regenerate. Reduced immunity, vitamin A deficiency, diabetes, and hormonal changes (for example, during menopause) disrupt tissue nutrition. Under these conditions, even minor trauma can lead to the appearance of keratinized areas on the mucous membrane.

Types and Locations of Leukoplakia

Types and locations of leukoplakia

The appearance and treatment strategy largely depend on the location of the pathological lesion: this is why diagnostics and testing are necessary.

Oral leukoplakia

It is most often found on the buccal mucosa (along the line where the teeth meet), on the tongue, or in the corners of the mouth. Oral leukoplakia appears as grayish-white patches with sharp or blurred edges.

Cervical leukoplakia

This form is often completely asymptomatic and is only discovered in a gynecologist's office. Cervical leukoplakia is still a dense white spot. It is important to understand that without colposcopy and cytology, it is impossible to determine whether there is dysplasia (a dangerous change in cell structure) or simply a protective reaction of the epithelium.

Vulvar leukoplakia

The tissues of the external genitalia become dry, lose elasticity, and become covered with a whitish coating. Severe itching and burning occur. This leads to depression and despair.

Other forms and locations

There is also leukoplakia of the bladder and larynx. These are less common.

In the first case, the person experiences pain similar to cystitis, while in the second, the voice becomes hoarse. Any changes in the mucous membrane that persist for more than two weeks should be examined by a doctor.

Symptoms of leukoplakia

Leukoplakia is virtually painless in its early stages. Many people discover the condition during a medical examination.

General signs

The main sign is white spots on the mucous membrane. They come in various shapes and textures:

  • Flat and smooth (simple form)
  • Bumpy, raised above the surface (warty form)
  • Covered with cracks and ulcers (erosive form)

Symptoms by location

Patient complaints vary depending on the organ affected. If the mouth is affected, a burning sensation may occur when eating spicy foods. If the genitals are affected, itching and a feeling of tightness may occur. If the urinary tract is affected, discomfort occurs when urinating.

Symptoms of leukoplakia

General information

What is the danger of leukoplakia?

The main risk is its development into cancer. Leukoplakia itself is a benign process, but it creates the basis for cancer.

When the risk of complications increases

The likelihood of developing into cancer is higher if:

  • The lesion is located on the tongue or floor of the mouth
  • Erosions or cracks appear on a white background
  • The spot is rapidly increasing in size
  • The person continues to smoke or is exposed to harmful factors

Why are biopsy and histology important?

A doctor's visual examination can only suggest a problem. The eye cannot see what is happening inside the cells. Only a biopsy of leukoplakia and subsequent histology provide an answer to the main question: whether there are atypical cells in the tissue. The results of this analysis determine the entire subsequent strategy: whether simple observation is possible or whether urgent removal of the lesion is necessary.

What is the danger of leukoplakia?

Diagnosing leukoplakia

Modern diagnostics of leukoplakia is a multi-step process that allows us to rule out similar conditions (for example, candidiasis or lichen planus).

Examination and medical history

The doctor determines when the stain appeared, whether there are any bad habits or concomitant illnesses. It is important to identify the "chronic trauma" that may have triggered the process. For example, the dentist will definitely check the condition of fillings and crusts.

Instrumental methods

For a detailed examination, magnifying equipment is used. In gynecology, this is colposcopy (examination of the cervix under a microscope), and in dentistry, an examination using special lamps (autofluorescence stomatoscopy), which highlight suspicious areas.

Laboratory tests

The patient may be prescribed:

  • Cytological smear (cell scraping from the surface)
  • Tests for HPV and other infections
  • General blood tests to detect systemic inflammation

Biopsy and histological examination

This is the most important step. The doctor takes a small piece of tissue from the most affected area. Histological examination confirms the diagnosis of leukoplakia and determines the degree of risk (the presence or absence of dysplasia).

Diagnosing leukoplakia

Treatment of leukoplakia

The approach is always individual, so leukoplakia treatment begins with a comprehensive approach to health.

Eliminating the cause and risk factors

It's pointless to treat the mucous membrane if the irritant remains.

  1. Replacing sharp fillings and adjusting dentures
  2. Completely abstaining from smoking and strong alcohol
  3. Treating underlying conditions (gastritis, diabetes)

If the cause of the injury is eliminated, the stain may disappear after some time.

Conservative Treatment

If the histology does not reveal any dangerous changes, topical treatment is prescribed: oil-based vitamin solutions, keratoplasty (medications that accelerate healing), and anti-inflammatory medications.

The goal of this treatment is to soften the keratinized area.

Removal of the affected areas

If dysplasia is detected or conservative treatments are ineffective, the area must be removed. Today, gentle methods are used for this:

  • Laser destruction
  • Radio wave method
  • Cryodestruction (freezing with liquid nitrogen)
  • Photodynamic therapy

Post-treatment follow-up

Post-treatment follow-up is mandatory. The patient must return for check-ups every 3-6 months so the doctor can detect any relapses early.

Oral cavityWhite plaques, roughness, discomfortExamination, biopsy as indicatedTreatment of dental trauma, local therapy, laser
CervixOften asymptomatic, white spot in the speculumColposcopy, cytology, biopsyTreatment of infections, radio wave removal of the lesion
VulvaItching, dryness, whitish rashExamination, vulvoscopy, biopsyOintments with vitamins and hormones, removal of the lesion

Prevention and Lifestyle Recommendations

The disease may return. To prevent this, give up bad habits and strengthen your immune system.

How to Reduce the Risk of Recurrence

The main goal of prevention is to protect the mucous membranes. Avoid excessively hot and spicy foods, which can cause chemical burns. Maintain oral hygiene and visit your dentist regularly. It is important for women to monitor their hormonal levels and promptly treat pelvic inflammation.

Why are follow-up examinations important?

Leukoplakia is a chronic disease. Even if all is well, the inflammation can return with the first weakening of the immune system. A follow-up examination is the only way to detect the moment when a precancerous condition develops into cancer.

Prevention and lifestyle recommendations
When to see a doctor as soon as possible

There are certain signs that indicate you shouldn't "wait weeks" for an appointment. Seek immediate help if:

  • A white spot or plaque on the mucous membrane has begun to grow rapidly.
  • The area has become dense and hard to the touch.
  • Cracks have appeared that bleed or don't heal.
  • Constant pain or severe itching has developed.
  • The lesion has changed color (red or dark spots have appeared).

FAQ

Can leukoplakia resolve on its own?

If the traumatic factor is completely eliminated, leukoplakia of the mucous membrane may resolve without treatment. However, the spot still needs to be monitored.

Is a biopsy necessary?

If you have white spots on your cervical mucosa, a biopsy is necessary. It's also necessary if the spot doesn't go away for more than two weeks.

Is leukoplakia contagious?

No, it is not contagious. It cannot be spread through kissing, hugging, or sharing utensils.
Conclusion

If you ignore the symptoms of leukoplakia, the condition can develop into cancer. Remember, your health is your responsibility.
