A child's medical record, Form 026/u, is an official document confirming their health and readiness to attend kindergarten or school. Form 026/u is issued based on the results of a complete medical examination and includes information about past illnesses, vaccinations, physical development, reports from specialized specialists, and the child's final admission to kindergarten and school. The medical examination is conducted according to an approved list, which allows for an assessment of overall health and the identification of possible contraindications to physical activity. This document is mandatory for enrollment in a preschool or educational institution and is accepted only in the prescribed form. At the K+31 Clinic, the preparation of Form 026/u medical record is carried out in accordance with the requirements of the Ministry of Health, with the participation of qualified doctors and the ability to complete all examinations in one day.

The medical record is an A4 notebook that will contain information about the child throughout their entire time attending educational institutions (from enrollment in kindergarten until graduation).

Structure of form 026/u

The structure of medical record form 026/u is developed according to uniform requirements and includes several mandatory sections reflecting all information about the young patient's health. The title page contains basic information about the child—last name, first name, date of birth, residential address, parents' information, and compulsory medical insurance policy number. This is followed by a medical history section, which records information about past illnesses, chronic conditions, and individual developmental characteristics. A separate section is dedicated to vaccination history: it lists vaccination dates, medication names, and reactions to them.

An important part of form 026/u consists of reports from specialized specialists—pediatricians, ophthalmologists, otolaryngologists, surgeons, neurologists, dentists, and other physicians—as well as the results of laboratory tests and diagnostic studies. The final section contains recommendations for monitoring and physical activity. Medical record 026/u is kept at the educational institution and updated as needed to reflect current information about the child's health.

Document mandatory

According to the current admissions procedure for educational institutions, parents are required to provide a medical certificate to the institution. Failure to submit the documents required for enrollment is grounds for denial of admission.

The medical certificate is typically issued in the form of a card. Legislation does not prohibit the submission of a medical commission's report containing information on admission to kindergarten in a different form. In this case, the educational institution to which the student is applying must issue the card.

What doctors do I need to see?

The list of specialists may differ slightly, this is due to the fact that the age of children entering an educational institution for the first time is different. Some children do not attend kindergarten and therefore are faced with the registration of a card only when entering school. An extended list of doctors is also possible for admission to correctional institutions.

Pediatrician

The pediatrician conducts an initial examination and anamnesis, issues referrals for tests and makes appointments with specialists.

Based on the results of the medical examination, issues a conclusion on the child's readiness to attend an educational institution, determines the health group, and introduces parents to the doctors' recommendations.

Children's neurologist

The doctor will determine the child's level of psychological maturity, check the vestibular system, and determine the level of mental development in accordance with the age norm.

Pediatric ophthalmologist

If necessary, will measure intracranial pressure, check visual acuity, determine the presence/absence of problems leading to the development of strabismus and astigmatism. It is in childhood that identified problems can be treated, which is much more difficult in adulthood.

Dermatologist

A specialist examination will allow you to check for skin diseases (eczema, psoriasis, scabies, etc.), which is especially important in the case of a contagious dermatological disease.

Pediatric surgeon/Orthopedic surgeon

Will assess the child's developed musculoskeletal system and musculoskeletal system. The examination allows for early detection of tumors and hernias.

Children's otolaryngologist

Will assess the anatomical structure of the ENT organs, check the child's hearing level. In childhood, there is a high risk of developing adenoids, examination and recommendations of a specialist will help prevent the occurrence of this disease.

Pediatric dentist

To determine the child's psychological readiness for further schooling, a specialist examination before school includes a conversation and a test assignment.

Tests and examinations for medical record form 026/u

To obtain a medical record form 026/u, a child must undergo a standard set of laboratory tests and instrumental examinations.
