Preparation for consultation
Dear Patients!
To make your consultation with a specialist go through more fully and in detail, we recommend you:
- Bring printed medical records with you, especially if the results are from other institutions.
- If you need a sick leave, then in the clinic we will request the data of SNILS.
When you first contact the clinic, you will need passport data to conclude a contract.
If the payer and the patient are different people, the payer's data is also required.
