About the disease

Onychocryptosis, or as it is often called, ingrown toenail, is a typical disease in the field of podiatry. When the nail plate grows into the lateral or distal area of the nail fold, swelling and inflammation of the soft tissue occurs. As a result, severe pain occurs and sometimes pus forms. The most common ingrown toenail is the big toe.

This condition usually occurs due to improper pedicure, cutting the free edge of the nail and wearing tight shoes. It can also be the result of injuries that change the direction of growth of the nail plate. It has been noted that people with flat feet, especially those with hallux valgus, are most prone to developing onychocryptosis.

Therapy of onychocryptosis always requires a qualified approach. If left unattended, serious complications are possible, including purulent inflammation and sepsis. Therefore, the disease requires professional treatment.

Pain, inflammation and secondary infection are just some of the symptoms caused by an ingrown edge of the nail plate into the soft tissue of the finger. If you neglect this condition and do not seek medical help at the right time, the consequences can be quite serious - in severe cases, loss of a finger occurs.

  • Improper nail care. A round or too short manicure can stimulate ingrown nails
  • Narrow shoes. Shoes and boots that pinch your toes and nails can cause tissue damage. Blows, abrasions or regular rubbing of the feet often provoke ingrowth
  • Sometimes onychocryptosis is inherited. In this case, its development is due to the structural features of the nail bed

Symptoms and diagnosis

Symptoms associated with ingrown toenails range from mild discomfort to severe pain. Additional features include:

  • Redness of the skin around the nail.
  • Swelling
  • Purulent discharge.
  • Growth of granulations - "fleshy growths" around the nail.

Accurate and timely diagnosis of an ingrown toenail helps avoid further complications and speeds up the recovery process. It includes a visual examination and medical history. In some cases, additional testing (x-rays) may be required. This helps rule out other problems, such as fractures or bone infections.

Treatment methods

How to treat an ingrown toenail? The choice of method depends on the stage of the disease. In more severe cases, surgical treatment is performed. In this case, part or all of the nail is removed under local anesthesia.

Conservative treatment. For mild ingrown nails, local antiseptics, baths, lotions and orthopedic products (plasters, nylon threads, silicone gaskets) are used.
Minimally invasive methods. It is expected to use laser, radio waves, and cryodestruction.

Treatment with conservative methods

Ingrown toenail treatment is carried out using springs, staples and plates that are attached to the nail plate. They imperceptibly lift the ingrown area and gradually correct the deformation. Despite their miniature size, these devices do not create discomfort while wearing shoes.

Special ointments and balms are also used during the treatment process, which help soften the nail, relieve inflammation and reduce pain.

Of course, an important part of the treatment process is proper nail cutting and wearing comfortable shoes that do not put pressure on the toes. When manicuring, you should use scissors with straight blades. You don’t need to cut your nails too short – just to the level of your fingertips. If sharp edges remain, they should be carefully filed to avoid skin irritation.

Preparing for the procedure for removing an ingrown toenail

How to treat an ingrown toenail if nothing helps? In this case, an operation is performed. Preparation for the procedure includes:

  • Carrying out analyses. A general blood test, biochemistry, and testing for the presence of HIV, hepatitis and syphilis are prescribed.
  • Stop taking certain medications. Anticoagulants may increase the risk of bleeding, so their use is temporarily discontinued.
  • Skin preparation. Local use of antiseptics and ointments is expected. This is necessary to reduce the risk of infection.

After careful preparation, immediate removal of the ingrown toenail follows.

Removal process

How to remove an ingrown toenail? The surgical treatment process includes the following stages:

  • Pain relief. A ring block is used without the use of adrenaline. The patient is then placed in the supine position with the catheter at the base of the thumb.
  • Performing an operation. First, a wedge-shaped excision is performed. Further steps: making an incision, excision of the skin-nail and suturing.
  • Measures for specific complications. In some cases, complete removal of the nail and its bed may be necessary.

The whole process takes about 30-40 minutes. After surgery, sterile dressings are applied to the nail bed. Sutures are removed 10 days after surgery. The average period of incapacity for work is about 2 weeks.

Postoperative period and rehabilitation

Proper foot care after removal of an ingrown toenail is important for quick recovery and prevention of relapse of the disease.

In the postoperative period, the following points are important:

  • Regularly change dressings and treat the wound with antiseptics.
  • Refusal to wear narrow and uncomfortable shoes.
  • Following the rules of hygiene and proper nail care (this helps prevent re-ingrowing).
