Medical card 026/u for school or kindergarten - registration on the day of application

Before your child enters school or kindergarten in K+31, you can see all the specialists in 2 days or bring the completed reports to issue the card on the day of application.

What is included in the registration:

  1. Child's medical record in form No. 026/u
  2. Examination by a pediatrician and reports from specialists (ophthalmologist, surgeon, ENT, dentist, etc.)
  3. Results of tests and examinations
  4. Conclusion on the health group and physical education group
  5. Assessment of the child's readiness to attend school or kindergarten

To issue the card, you must provide information about vaccinations and the results of tuberculin diagnostics for the last year.

For your convenience, there is a “box” version of a medical examination before school or kindergarten.
