Silent sinus syndrome (SOS) includes a painless involution of the maxillary sinus (CSF) due to occlusion of the ethmoid funnel (the formation of the lateral nasal wall, refers to the anterior ethmoid bone), accompanied by enophthalmos, in contrast to simple maxillary atelectasis. The literature describes no more than 100 observations.

Distinguish between primary (spontaneous) and secondary SMS.

Symptoms of Silent Sinus Syndrome

The symptom is only the gradual development of enophthalmos (deepening of the eyeball) on one side. It usually develops for a long time. Primary, or spontaneous, SMS occurs in adulthood, with the first symptoms occurring over several months or years. The reasons for the development of secondary SMS are facial trauma, chronic rhinosinusitis, functional endoscopic endonasal interventions.

Diagnostics

The gold standard in the diagnosis of SMS is CT of the paranasal sinuses, which provides high detailing of bone structures, in contrast to MRI. However, even on a standard x-ray of the paranasal sinuses, darkening and a decrease in the volume of the maxillary sinus can be determined, which will give us grounds for further examination.

Silent Sinus Syndrome Treatment

Surgical only. It consists in creating a stable communication between the sinus and the nasal cavity. The operation is possible only with the use of endoscopic techniques and microinstrumentation. During the operation, the qualification of the surgeon is important - due to the prolapse of the orbital wall, the anatomical relationships change and it is very easy to damage the orbit. When air exchange is restored, enophthalmos regression, as a rule, occurs. If the effect is insufficient, the second stage is the plastic of the lower wall of the orbit.
