The Clinic has a parking lot. To order a pass, you need to call 8-800-7773131 or write to WhatsApp +7 (915) 493-95-92 and leave the data: name, make and car number. You can also leave these data on the website through an electronic appointment form for an appointment with a doctor.

The availability of parking spaces depends on the time of the visit and the traffic situation.

2. WHAT DOCUMENTS TO TAKE WITH YOURSELF?

1. IN PRIMARY CONSULTATION IN THE POLYCLINIC:

1. Patient's passport.
2. Payer passport (if payment is made by a third party).
3. It is advisable to have the previous medical documentation with you (extracts, research results, analyzes, ECG, images, disks).
4. In connection with the amendments to 323 Federal Laws that have entered into force regarding the need to issue an electronic certificate of incapacity for work, SNILS data is required when issuing a medical card.

2. WHEN HOSPITALIZATION IN THE HOSPITAL:

1. Patient's passport.
2. Payer passport (if payment is made by a third party).
3. With planned hospitalization by surgical profile - the results of preoperative preparation.
4. It is advisable to have with you previous medical documentation (extracts, research results, analyzes, ECG, images, disks).
